How insane can someone be if they are able to realize that they're insane?

I just wanted to point out that John Nash did NOT become self aware without the help of professional involvment..... he was involuntarily committed to a hospital and still did not think he was insane. He was deeply involved with professionals when he finally understood that his hallucinations were not real. Bad example.
